Invisible Air Pollution May Be Causing Nearly 2 Million Early Deaths a Year, Scientists Warn

The world's tiniest air pollution particles—so small they can slip through the lungs into the bloodstream and even reach the brain—may be responsible for nearly 1.99 million premature deaths every year, according to a major international study that is raising fresh concerns about an overlooked public health threat.

Unlike PM2.5, the fine particles already regulated in many countries, ultrafine particles (UFPs) remain largely unregulated despite mounting evidence of their health risks. These particles are thousands of times thinner than a human hair and are produced mainly by vehicle exhaust, industrial emissions and fossil fuel combustion. Because of their tiny size, they can evade the body's natural defences, travel throughout the body and trigger disease.

Researchers led by Prof. Dr Jos Lelieveld, a former director at the Max Planck Institute for Chemistry, made the first detailed global map showing long-term exposure to ultrafine particles by using satellite data, information about land use, machine learning, and measurements from 155 locations around the world collected between 2010 and 2019. The study estimates 1.99 million annual premature deaths, with the true figure likely ranging between 810,000 and 3.89 million, reflecting statistical uncertainty.

"Ultrafine particles are literally a blind spot in air quality policy: they are not covered by any regulation, even though they are ubiquitous in our cities. With our data, we can for the first time show worldwide where exposure is highest and which sources are responsible – above all, combustion in traffic, industry and energy production. This gives policymakers a concrete tool to take targeted action," Lelieveld said.

These particles can damage blood vessels, increase harmful chemical stress inside cells, promote artery blockage and raise the risk of heart attack, stroke and heart failure.

"What is particularly alarming for us in cardiology is that ultrafine particles bypass the body's natural protective barriers and can reach the bloodstream—and even the brain—directly... The findings have particular relevance for India. A 2025 study by Rajeev Kumar Mishra of Delhi Technological University and others found that indoor ultrafine particle levels in Delhi homes reached 10⁴ to 10⁵ particles per cubic centimetre, with fireworks increasing concentrations six-fold. Cooking, dusting and even religious activities also released large numbers of these particles, while outdoor pollution carried toxic metals indoors, exposing families who spend nearly 80% of their time inside their homes.

The authors estimate that introducing an annual limit of 5,000 particles per cubic centimetre could reduce global excess deaths by about 45%. As the World Health Organization and the European Union now classify ultrafine particles as an emerging contaminant of concern, the study adds weight to calls for stricter monitoring and cleaner transport and energy systems before this invisible pollutant claims even more lives.
