Not Just How Much Fat You Carry — But Where: New Study Links Fat Distribution to Cancer Risk

For decades, doctors have cautioned that being overweight raises your risk of several cancers — from breast and bowel to liver cancer. But a new study is turning that belief on its head, revealing that where your body stores fat may be just as important as how much you have.


Published in the Journal of the National Cancer Institute, the study has uncovered a fascinating — and somewhat alarming — truth: fat isn’t created equal. While some fat types may increase cancer risk, others might actually protect against it.

BMI Isn’t the Full Story Anymore

Body Mass Index (BMI) has long been used as a quick way to determine obesity and related health risks. But scientists now argue it’s far too simplistic. Using a sophisticated genetic analysis method called Mendelian randomization, researchers examined fat distribution in five key areas — the belly, organs (visceral fat), thighs and hips, liver, and pancreas — and tracked their links to 12 obesity-related cancers.

The findings were eye-opening. Belly fat — long considered the “danger zone” — was strongly linked to higher risks of endometrial, esophageal, and liver cancers. In contrast, fat around the hips and thighs appeared to protect against breast cancer and even a rare brain tumor called meningioma.

“Obesity Location Matters”

“It’s becoming clear that not only does obesity matter — but where that obesity sits matters even more,” said Dr. Daniel Landau, a board-certified oncologist and hematologist. “Central obesity — that’s fat around the abdomen — is tied to poor dietary habits, inflammation, and hormonal changes that can fuel cancer growth.”

Dr. Anton Bilchik, surgical oncologist and Director of the Gastrointestinal and Hepatobiliary Program at Providence Saint John’s Cancer Institute, described the research as “methodologically groundbreaking.”

“The study uses genetic data to explore how fat in different areas of the body affects cancer risk,” he explained. “It’s complex science — but the message is simple: not all fat behaves the same.”

Good Fat, Bad Fat? It’s Complicated

The study found that abdominal, liver, and visceral fat increased cancer risk, especially for liver and endometrial cancers. However, gluteofemoral fat — the kind stored around your thighs and hips — had the opposite effect.

“This is a surprising and intriguing finding,” said Dr. Bilchik. “It suggests that fat in the thighs and hips could have protective effects, perhaps through hormone regulation, particularly estrogen.”

Why BMI Alone Can Mislead

The research underscores that BMI — a measure that simply compares height and weight — doesn’t tell the full story. Two people with the same BMI could have entirely different health risks depending on how their fat is distributed.

“BMI gives a very rough idea of someone’s health,” said Dr. Landau. “It’s quick and convenient, but it doesn’t reflect what’s really happening inside the body. Fat around the belly, for instance, is far more dangerous than fat on the thighs.”

The Bigger Picture

Scientists are now calling for a more personalized approach to obesity and cancer prevention — one that looks beyond numbers on a scale. The study shows that fat distribution affects cancer risk through complex biological pathways involving inflammation, hormones, and genetic factors.

While losing excess fat remains important, this research adds a new layer of understanding: where you carry fat could determine which cancers you’re more likely to develop — or avoid.
