Ceftriaxone with Vancomycin
Ceftriaxone and Vancomycin are antibiotics used in combination to treat a wide range of bacterial infections. Ceftriaxone is a cephalosporin antibiotic, and Vancomycin is a glycopeptide antibiotic. Together, they provide broad-spectrum coverage against various bacteria.-It is indicated in the following infections due to susceptible organisms. Pneumonia (Ventilated Associated Pneumonia, Hospital acquired Pneumonia, Bacterial Meningitis, Bacterial Endocarditis, Septicemia, Infections in immunocompromised patients. -Bacterial Infections: This combination is commonly used for severe bacterial infections, including pneumonia, meningitis, septicemia, and other complicated infections. -Mixed Infections: When the infecting bacteria involve both gram-positive and gram-negative organisms.
-Dosage depends on the specific infection, patient factors, and susceptibility of the causative bacteria. The antibiotics are typically administered intravenously under medical supervision. -Adjustment in Renal Impairment: Ceftiraxone is excreted via both biliary and renal excretion. Therefore, patients with renal failure normally require no adjustment in dosage when usual doses of ceftriaxone are administered. -Dosage adjustment with vancomycin must be made in patients with impaired renal function. There are no dosage reduction studies for the combination in renal impairment.
-Hypersensitivity: Patients with known hypersensitivity to ceftriaxone or vancomycin. -Concurrent Use with Calcium-containing Solutions: Ceftriaxone should not be mixed with or administered simultaneously with calcium-containing solutions due to the risk of precipitation.
-Renal Impairment: Adjustments in dosage may be required in patients with renal impairment. -Hematologic Monitoring: Regular monitoring of blood counts is recommended during prolonged therapy.
Common side effects may include gastrointestinal disturbances, rash, and pain at the injection site. Serious side effects are rare but may include allergic reactions, superinfections, and renal impairment.
-Probenecid: May increase and prolong the therapeutic levels of ceftriaxone. -Nephrotoxic Agents: Caution is advised when combining with other nephrotoxic drugs due to potential additive effects.
