Fosaprepitant

Fosaprepitant is a prodrug of aprepitant, which is a substance P/neurokinin 1 (NK1) receptor antagonist. It is used to prevent nausea and vomiting caused by chemotherapy, particularly in patients who are receiving highly emetogenic chemotherapy. It is prodrug of aprepitant, which is antagonist of human substance P/neurokinin 1 (NK1) receptors.

Indications

Prevention of acute and delayed nausea and vomiting associated with initial and repeat courses of highly emetogenic cancer chemotherapy.


Dosage

IV; 150 mg intravenously over 20 to 30 minutes approximately 30 minutes prior to chemotherapy.


Contra-Indications

Hypersensitivity to fosaprepitant or any component of the formulation.


Special Precautions

Paediatric: safety and efficacy not yet established. Pregnancy: Category B. Lactation: Data not sufficient. Elderly:Use with caaution.


Side Effects

Common side effects may include fatigue, hiccups, constipation, diarrhea, and injection site reactions. Less common but more severe reactions can include hypersensitivity reactions and changes in liver function tests.


Drug Interactions

Fosaprepitant may interact with drugs that are metabolized by CYP3A4, potentially increasing their plasma concentrations. Concomitant use with such drugs should be monitored closely.
