Bosentan
It is an Endothelin Receptor Antagonist.Used in treatment of Pulmonary Hypertension. It is also used for reducing the number of new digital ulcers in systemic sclerosis and digital ulcer disease.
Initial dosage of 62.5mg twice daily for 4 weeks, later increased to a maintenance dose of 125mg twice daily.
It is contraindicated in pregnancy hepatic dysfunction and in patients taking glyburide, Cylosporin A. Hypersensitivity to Bosentan.
Administration of this medicine can lead to liver injury, fluid retention, decreased sperm counts, decreased hemoglobin and hematocrit. Nursing mothers are supposed to discontinue either taking the medication or brest feeding depending on mother's condition.
Headache, flushing, itching, runny nose, sore throat, heart burn, dizziness. Serious side effects may include fast/pounding heartbeat, extreme tiredness, swelling of ankles/feet, unexplained/sudden weight gain.
Drugs modifying liver enzymes-azole antifungals (Ketoconazole, Itraconazole), macrolide antibiotics (Erythromycin), amiodarone, tacrolimus, frosamprenavir, carbamazepine, rifampicin. Bosentan can enhance the removal of certain drugs from body like-statins, anticoagulants (warfarin), hormonal contraceptives.
