Diphenylmethanes (Bisacodyl)

(Phenolphthalein, Bisacodyl):- They are partly absorbed and re-excreted in bile: entero-hepatic circulation is more important in phenolphthalein, which can produce protracted action. Diphenylmethanes, commonly known as Bisacodyl, belong to the class of stimulant laxatives. These agents act directly on the intestinal mucosa, promoting bowel movements by stimulating colonic peristalsis.

Indications

-All forms of constipation, e.g. in bedridden patients, due to change of food or environment, illness or digestive disorders. Relief of evacuation in painful conditions such as haemorrhoids. Pre-and postoperatively. Preparation for barium enema; preparation of colon for proctosigmoidoscopy. -Bisacodyl is primarily indicated for the treatment of constipation and to prepare patients for certain medical procedures, such as colonoscopies.


Dosage

-Bisacodyl : 5-15 mg at bedtime.Phenol-phthalein: 60-130 mg at bedtime. -The dosage of Bisacodyl varies depending on the specific formulation (oral tablets, suppositories, etc.) and the individual patient's needs. It is crucial to follow the prescribed dosage and administration instructions provided by the healthcare professional.


Contra-Indications

-Bisacodyl is contraindicated in patients with a known hypersensitivity to the drug, intestinal obstruction, severe abdominal pain, and inflammatory bowel diseases such as Crohn's disease or ulcerative colitis. -Acute surgical abdomen.


Special Precautions

-Caution is advised in patients with electrolyte imbalances. -Use with care in elderly patients and those with impaired renal or hepatic function. -Avoid prolonged use, as it may lead to laxative dependence. -First 3 months of pregnancy. The concurrent administration of antacids (Sod. bicarbonate) and milk should be avoided.


Side Effects

-Common side effects may include abdominal cramps, diarrhea, and electrolyte imbalance. In rare cases, individuals may experience allergic reactions such as rash or itching. If any unusual or severe side effects occur, medical attention should be sought. -Abdominal discomfort. Soreness in the anal region due to leakage of the suppository base.


Drug Interactions

Bisacodyl may interact with certain medications, including antacids, diuretics, and other laxatives. Patients should inform their healthcare provider of all medications they are taking to avoid potential interactions.
