Fosfomycin Sodium

It is bactericidal and inhibits bacterial cell wall biogenesis by inactivating the enzyme UDP-N-acetylglucosamine-3-enolpyruvyltransferase, also known as MurA. Fosfomycin has broad antibacterial activity against both Gram-positive and Gram-negative pathogens, with useful activity against E. faecalis, E. coli, and various Gram-negatives such as Citrobacter and Proteus.

Indications

Osteomyelitis, Complicated urinary tract infections, Nosocomial lower respiratory tract infections, Bacterial meningitis, Bacteraemia.


Dosage

IV; daily dose of fosfomycin is determined based on the indication, severity and site of the infection, susceptibility of the pathogen(s) to fosfomycin and the renal function. In children, it is also determined by age and body weight, 12 to 24 gm in 2-3 divided doses.


Contra-Indications

Hypersensitivity.


Special Precautions

Paediatric: Use with caution in infants taking renal function into consideration. Pregnancy: Data not sufficient. Lactation: It is found in human milk, it should therefore not be administered during lactation, unless the benefit outweighs the risk. Elderly: Use with caution taking renal function into consideration.


Side Effects

Retching, stomach ache, Injection site phlebitis, Decreased appetite, hypernatraemia and/or hypokalaemia, Dysgeusia, headache, Vertigo, Dyspnoea, Blood alkaline phosphatase, aspartate aminotransferase and alanine aminotransferase increased (transient), rash, fatigue.
