SALMETEROL

It is a selective long acting beta -2 adrenoceptor agonist and are used in the treatment of reversible obstructive pulmonary diseases such as asthma, emphysema, and bronchitis because they produce bronchodilation, inhibit the release of inflammatory mediators from mast cells and increase ciliary motility.

Indications

Reversible airways obstructions in asthma.


Dosage

Adults: 2 inhalations(50 microgram) twice daily. Severe airway obstruction: 4 inhalations (100 micrograms) twice daily.


Contra-Indications

Hypersensitivity.


Special Precautions

Patients response should be monitored clinically and by lung function tests.It is not a replacement for oral or inhaled corticosteroids but use is complimentary to them.It is not designed to relieve acute asthmatic symptoms. Paediatrics: Not recommended under 4 years. Pregnancy: Use only, if clearly indicated. Lactation: Use only, if clearly indicated. Elderly: Safe.


Side Effects

Tremor,tachycardia,subjective palpitations and headache.


Drug Interactions

Hypokalemia with steroid or diuretics. Effects are antagonised by non-selective betablockers and less so by cardioselective betablockers. Potentiation of vascular effects with MAOIs and tricyclic antidepressants.
