Topiramate

Topiramate

Used as adjunctive treatment of seizures including refractory seizures, simple and complex partial seizures.

Indications

Adjunctive treatment of partial seizures with or without secondary genaralisation and when not well controlled by other drugs. Seizure associated with Lennox Gastaut syndrome. Primary generalised tonic-clonic seizures.


Dosage

Adults: Start with 25mg daily for 1 week. Increase by 25-50mg daily at intervals of 1-2 weeks, going to a maximum dose of 800mg daily in 2 divided doses. Usual dose 200-400mg daily in 2 divided doses. Children: 2-16 yrs. : Initially 25mg at night for 1 week and then increase in steps of 1-3mg/kg daily as per response in intervals of 1-2 weeks; taken in 2 divided doses. Recommended dose 5-9mg/kg daily in 2 divided doses.


Contra-Indications

Lactation.


Special Precautions

Abrupt drug withdrawl, pregnancy, hepatic impairment or renal impairment. Ensure proper hydration.


Side Effects

Abdominal pain, anorexia, nausea, weight loss. Impaired concentration and memory, confusion, emotional lability, mood disorders, depression, impaired speech, altered behaviour. Abnormal gait, ataxia. Dizziness, drowsiness, paresthesia, fatigue, asthenia. Migraine, nystagmus, Visual disturbances, diplopia, taste disorder. Aggression psychotic symptoms, cognitive problems. Leucopenia.
