Nicorandil
-Nicorandil is a medication primarily used to treat angina pectoris, a condition characterized by chest pain or discomfort due to insufficient blood flow to the heart muscle. It belongs to a class of drugs known as potassium channel openers. Nicorandil works by dilating blood vessels, improving blood flow to the heart, and reducing the workload on the heart. -Nicorandil is a vasodilatory drug that functions through potassium channels and intracellular cGMP concentrations commonly used to treat angina.-Angina pectoris (stable angina) -Chronic stable angina -Indicated for the prevention and treatment of chronic stable angina pectoris and reduction in the risk of acute coronary syndromes.
-The dosage of nicorandil may vary depending on the patient's condition. It is important to follow the doctor's prescription. Typically, the recommended starting dose is 10 mg twice daily, and the dosage may be adjusted based on the individual response. -Oral; 10 to 20 mg twice a day maximum upto 40 mg. In patient prone to headache, reduced doe of 5mg is given.
Nicorandil is contraindicated in individuals with: -Hypersensitivity to nicorandil or any of its components. -Severe hypotension (low blood pressure). -Shock, including cardiogenic shock. -Acute myocardial infarction (heart attack). -Obstructive conditions of the heart, such as aortic or pulmonary stenosis. -Hypersensitivity, Cardiogenic shock; hypotension, left ventricular failure with low filling pressure.
-Patients with impaired kidney or liver function should use nicorandil with caution. Use cautiously in patients with a history of hypotension or those prone to dehydration. Caution is advised in elderly patients. Nicorandil may cause headaches, and patients should be cautious when driving or operating machinery. Paediatric: Safety and efficacy in pediatric patients have not been established Pregnancy: Data not sufficient Lactation: Data not sufficient Elderly: Use with caution
-Common side effects may include: Headache Flushing Dizziness Nausea -Serious side effects are rare but may include: Severe hypotension Ulcerations in the gastrointestinal tract Blood disorders -Headache (usually transitory), flushing, dizziness, nausea, vomiting and weakness. Hypotension and reflex tachycardia at high doses.
-Concomitant use with other vasodilators may increase the risk of hypotension. -Avoid concomitant use with phosphodiesterase inhibitors (e.g., sildenafil) due to the risk of severe hypotension. -Use with phosphodiesterase 5 inhibitors, e.g. sildenafil, tadalafil, vardenafil, is contraindicated, use with soluble guanylate cyclase stimulator (such as riociguat) is contraindicated, NSAIDS, other antihypertensive drugs
