Immunoglobulins

These are preparations containing antibodies against infectious microorganisms and are usually prepared from human plasma or serum. Normal immunoglobulins are prepared from material from blood donors and contain several antibodies against infectious diseases prevalent in general population. Specific immunoglobulins contains minimum specified levels of one antibody. Anti-D immunoglobulines are given to prevent the formation of rhesus antibodies in rhesus-negative (Rh-ve) persons on exposure to rhesus positive red blood cells. Special precautions: If immunoglobulins are given after a live vaccine and interval of 3 weeks should be allowed to elapse. A live vaccine should be after and interval of 3 months of immunoglobulin administration.

Indications

Immunodeficiencies, Kawasaki disease, Immune thrombocytopenic purpura, Guillain-Barré syndrome, Prevention of graft-versus-host disease and infections in patients submitted bone marrow transplantation, Children with congenital HIV and severe recurrent infections.


Dosage

Carimune(R): 400 mg/kg, IV, daily for 2 to 5 consecutive days -Initial infusion rate is 0.5 mg/kg/min for 30 minutes. -If initial infusion is tolerated, may increase to 1 mg/kg/min, and gradually increased step-wise as tolerated. -Maximum infusion rate is 3 mg/kg/min.


Contra-Indications

(1) IVIg is contraindicated in patients who have had an anaphylactic or severe systemic reaction following previous administration of gamma globulin, anti-immunoglobulin A antibodies, or thimerosal. (2) IVIg is relatively contraindicated in patients with isolated IgA deficiency. If IVIg treatment is deemed necessary, such patients should receive products containing the lowest IgA concentration possible and should be monitored closely for anaphylaxis.

Side Effects

Headache, dizziness, chills, muscle cramps, back or joint pain, minor chest pain, fever, nausea, vomiting, tiredness, or injection site reactions (pain, redness, and swelling).
