Neostigmine
They inhibit the cholinesterase enzyme which breaksdown acetylcholine in the synaptic cleft,thus increasing both the amount and duration of action of the transmitter at the receptor sites. ue to this action they help increase neuromuscular transmission which happen to be of great use in myasthenia gravis. Neostigmine is poorly absorbed orally : Oral dose is 20-30 times higher than parenteral dose & do not effectively penetrate cornea or cross blood-brain barrier. It is partially hydrolysed and partially excreted unchanged in urine.Myasthenia gravis.
15-30 mg daily or as required 1-5 mg i.v. with atropine 0.4-1.25 mg.
Urinary blockage. G.I. organic obstruction.
Cholinergic weakness. Bronchial asthma, cardiac disease, epilepsy. Parkin-sonism. Vagotonia.
Salivation, sweating, copious tracheo-bronchial secretions, miosis, blurring of vision, nausea, breathlessness, colic, involuntary defecation & urination.
