Paroxetine

Paroxetine is a selective serotonin reuptake inhibitor (SSRI) used as an antidepressant and anxiolytic medication. It works by increasing the levels of serotonin, a neurotransmitter in the brain, which helps in regulating mood. Paroxetine is commonly prescribed to treat major depressive disorder, obsessive-compulsive disorder, panic disorder, social anxiety disorder, generalized anxiety disorder, and post-traumatic stress disorder.

Indications

Paroxetine is indicated for the treatment of the following conditions: 1. Major depressive disorder (MDD) 2. Obsessive-compulsive disorder (OCD) 3. Panic disorder 4. Social anxiety disorder 5. Generalized anxiety disorder (GAD) 6. Post-traumatic stress disorder (PTSD)


Dosage

Depression: Initially 20mg daily, increased gradually, if necessary, by increments of 10mg to a maximum of 50mg daily. Obsessive-compulsive disorders: Initially 20mg daily, increase weekly in 10mg increments according to clinical response; the usual recommended maintenance dose is 40mg daily, although some patients may benefit from 60mg daily. Elderly patients, debilitated patients, and /or patients with severe renal or hepatic impairment: Initially 10mg daily increases may be made if indicate


Contra-Indications

Concomitant use in patients taking monoamine oxidase inhibitors (MAOIs).Thioridazine, Warfarin, Nonsteroidal anti-inflammatory drugs (NSAIDs)


Special Precautions

Patients with a history of mania and seizures. Close supervision of high risk patients with suicidal tendencies. Patients with severe renal or severe hepatic impairment. Avoid operating hazardous machinery, including automobiles. Abrupt discontinuation of paroxetine may lead to withdrawal symptoms, and gradual dose reduction is recommended when discontinuing the medication.


Side Effects

Asthenia, sweating, nausea, decreased appetite, somnolence, dizziness, insomnia, tremor, nervousness. Constipation, ejaculatory disturbance, and impotence.


Drug Interactions

Monoamine oxidase inhibitors : Avoid concomitant use since serious, sometimes fatal reactions have been reported. Paroxetine HCl should not be used in combination with a MAOI, or within 14 days of discontinuing treatment with MAOI. Warfarin : Concomitant administration should be undertaken with caution.
