PROGESTINS

These are the substances which convert the estrogen primed endometrium to secretory and maintain pregnancy. Progesterone is secreted by the corpus luteum in the later half of menstrual cycle under the influence of LH. Its production declines a few days efore the next menstrual flow. Main function is preparation of uterus for nidation and maintenance of pregnancy. The latter is probably due to decreased uterine motility & inhibition of immunological rejection of the foetus.

Indications

Contraceptive. Dysfunctional uterine bleeding. Endometriosis. Dysmenorrhoea. Premenstrual tension. Threatened/habitual abortion. Endometrial carcinoma. Infertility due to inadequate luteal phase.Adjunt to oestrogen replacement therapy.


Contra-Indications

Thrombophlebitis, thromboem- bolic disorders, undiagnosed vaginal bleeding. Hepatic impairment, cerebral apoplexy. Pregnancy. Dubin-Johnson, Rotor syndrome. Previous or existin liver tumours, history of jaundice or severe pruritus during pregnancy, history of herpes of pregnancy.

Side Effects

Nausea, breast enlargement, headache, migraine, rise in body temperature, mood swings. C.N.S. disturbances, urticaria, rash, acne, sweating, pruritus.
