Sevelamer Carbonate is a phosphate binder used to control hyperphosphatemia (elevated phosphate levels) in patients with chronic kidney disease on dialysis. It is a non-absorbed polymer that binds phosphate in the digestive tract and prevents its absorption into the bloodstream.

Indications

Hyperphosphatemia in patients with chronic kidney disease on dialysis.


Dosage

-Adults: The initial dose is typically 800-1600 mg three times daily with meals. The dose should be adjusted based on serum phosphorus levels, aiming to achieve a target range of 3.5 to 5.5 mg/dL. -Pediatric: The safety and efficacy in pediatric patients have not been established.


Contra-Indications

-Hypophosphatemia (low serum phosphorus levels). -Bowel obstruction.


Special Precautions

-Monitor serum phosphate levels regularly. -Caution in patients with dysphagia, swallowing disorders, severe gastrointestinal motility disorders (e.g., severe constipation), or major gastrointestinal surgery. -Vitamin and mineral supplementation may be necessary.


Side Effects

-Common: Nausea, vomiting, diarrhea, dyspepsia, abdominal pain. -Less common but serious: Bowel obstruction, ileus, and gastrointestinal perforation.


Drug Interactions

-Sevelamer Carbonate may affect the absorption of other medications. It is advised to take other medications at least one hour before or three hours after sevelamer. -Notable interactions include: -Ciprofloxacin: Reduced bioavailability. -Anti-arrhythmic and anti-seizure medications: Potential for decreased efficacy.
